What documents should I upload if I only take the SPI exam?

 : SPI 요구 사항에 따라 SPI 시험 응시를 신청하는 경우 다음 문서를 제출해야 합니다.

 1. 성적표 - 일반 대학, 의과대학 또는 초음파 대학, 고등교육 또는 대학교육 물리 수업(C 학점 이상)을 성공적으로 이수했음을 나타내야 합니다.

      *미국 또는 캐나다 이외의 학교에서 성적 증명서를 제출하는 경우 과목별 해외 성적 증명서를 제출해야 합니다.

  2. 또는 CME 인증서 - 일반, 의료 또는 초음파 물리학 세미나, 물리학 리뷰 과정 또는 물리학 통신 과정을 성공적으로 완료했음을 나타내야 하고, 최소 12 CME 학점 이상(ARDMS 승인)을 인정합니다.

      인증서는 ARDMS CME 요구 사항을 충족해야 합니다 (ARDMS 승인 CME를 보려면 www.ARDMS.org/CME 방문). CME 학점은 신청서 제출 전 2년 이내에 취득해야 합니다. (성적표와 CME 인증서 둘 중에 하나만 요구)

  3.  신분증 - 서명이 포함되어 있고 사진이 부착된, 만료되지 않은 정부 발급 신분증 사본; 신분증의 성과 이름은 ARDMS/APCA에 등록된 성과 이름과 정확히 일치해야 합니다.

     If you are applying to take the SPI examination under the SPI requirement you must submit the following documentation:

   1. A transcript (see transcript requirements below) reflecting successful completion of a graded general, medical or sonographic college, post-secondary or higher education physics class (with a grade of C or above);

    *If submitting a transcript from a school outside of the US or Canada, a formal course by course foreign transcript evaluation report must be submitted.

  2.  OR CME certificate denoting successful completion of a general, medical or sonographic physics seminar, physics review course, or physics correspondence course, denoting a minimum award of 12 ARDMS-accepted CME credits.

  The certificate must meet ARDMS CME documentation requirements (visit www.ARDMS.org/CME to view ARDMS accepted CMEs).

 The CME credits must be earned within two (2) years prior to application submission.

 3. AND Copy of a non-expired government-issued photo identification (ID) with signature; the first and  last names on the ID must exactly match the first and last names in your ARDMS/APCA record.
